Scope of Work
RE:
Sheen Asbestos will provide a licensed NYC Department of Environmental Protection (DEP) Asbestos Investigator and NYS-certified Environmental Inspector to examine the above property and the defined scope of work.
Services include:
•Field visit: site inspection by licensed professional (up to 1 hour on-site)
•Bulk material sampling with full chain of custody documentation
•Laboratory analysis via PLM and/or TEM through JLC Environmental Consultants
•ACP-5 filing with NYC DEP (if results negative and no ACM will be disturbed)
•Abatement survey letter per ASTM protocol (if results positive), for remediation vendor
•All documents signed and sealed — delivered approx. one week after field visit
Sampling involves non-invasive holes approx. the size of a quarter. Sheen Asbestos is not responsible for patching. Client must provide access to all areas in scope including mechanical rooms, public halls, and adjacent spaces as needed.

Sheen Asbestos is not responsible for work areas not included in scope drawings or amendments provided prior to field visit.

Client is responsible for ensuring full access on the scheduled date. Rescheduling fees may apply for missed access.

Results are specific to sampled areas and conditions at time of inspection. Sheen Asbestos is not liable for undiscovered ACM outside defined scope.

Amendments to scope must be provided in writing prior to field visit. Additional sampling or revised reports may incur additional fees.

NYC DOB permit or application number should be provided to Sheen Asbestos prior to inspection where applicable.

Sheen Asbestos carries workers' compensation and general liability insurance. Certificates available upon request.

By signing below, client acknowledges and accepts the scope of work, fee schedule, and terms outlined in this proposal. This document constitutes a binding agreement upon execution by both parties.
